Here is a copy of our program registration form and waiver.  Forms can be returned to the town hall at 125 Main St LIttleton, NH  03561  ATTN Chris Wilkins or Parks and Rec.   Please let us know if you have any questions.

Agree & Consent

Hold harmless agreement/Consent for medical treatment We the undersigned, hereby acknowledge that participating in sports and recreation programs with the Town Of Littleton can be dangerous for the participant. Injuries may occur as a result of participation, playing surface conditions, and others. We agree to hold harmless, the Town of Littleton, officials, sponsors, fellow participants, and any and all people involved from any claims or damages resulting from injuries while participating in an organized and supervised Recreational activity. Furthermore, as the parent or legal guardian of the above individual, I hereby give my consent for emergency medical care provided by a licensed EMT and or prescribed by a duly licensed Doctor of medicine, or Doctor of dentistry. This care may be given under whatever conditions necessary to preserve life, limb, or well being of my dependent or self. Periodically photographs will be submitted to local newspapers and posted on the Parks and Recreation Department’s website. This is done in an effort to keep families and community members informed of events and happenings.
